Back ache is the pain felt over back which is divided into neck pain, middle back pain or coccydynia based on the area affected. The lumbar area is the most commonly affected. It is a clinical condition which hinders working capacity of middle aged men or women.


There are many causes of back pain. But in most of the cases, the cause of back ache is non-specific origin, may be due to back strain or sprain. Some of the causes listed are herniated disc or stenosis, degenerative changes, traumatic attack on vertebrae, Metastatic cancer, osteomyelitis, epidural abscess. In some cases, the acute or sub- acute back ache may end up in chronic pain.

Poor nutrition, fatigue, sedentary lifestyle, obesity, heavy weightlifting, lack of exercise, smoking, psychological factors such as job stress, family issues etc, poor posture, pregnancy, after delivery are some of the risk factor for back pain.

History of cancer, fever, unexpected weightloss, immunosuppression, urinary infection, retention or incontinence, faecal incontinence, bilateral weakness of legs etc are causes or associated symptoms which signals serioud underlying pathology. These need proper diagnosis and emergency management if needed.



Degenerative or traumatic changes formed in discs are one of the most common causes. Inflammation or any diseases related to Gall bladder, pancrease, aorta and kidneys may also cause referred pain in the back.

Inflammatory changes, tumour of vertebrae, neural tissues and adjacent structures also manifests back ache. Some of the inflammatory changes may be autoimmune as in Psoriatic arthritis, RA, SLE, Ankylosing spondylosis.


Signs & symptoms

Characteristic features of back ache depend on the severity and origin of cause. Back pain may be acute, sub-acute or chronic depending on the duration of onset. Some of the features of back ache may be characteristic dull, shooting or piercing ache in the adjacent muscles and nerves, burning sensation, discomfort, numbness, weakness, referred or radiating pain to the bones, joints and extremities.



Bach pain is basically classified on the duration namely; Acute (<6weeks), Subacute (6-12 weeks) and Chronic (> 12weeks)


Ayurvedic Management of Back Ache

In Ayurveda Backache generally come under Vata disorders. Ayurveda internal and external treatment successfully manages Back ache due to inflammatory changes and traumatic changes in spine. Other back ache with an underlying causes or disease may be taken into consideration about their severity and chronicity.
